Changes in Phoria and Fusional Vergence Amplitude according to a Short-term Monocular Occlusion

Abstract
Purpose: To evaluate the effect of monocular occlusion time on phoria and fusional vergence.

Methods: Thirty adults (mean age 21.47¡¾1.83 years) participated in this study. All subjects measured phoria and fusional vergence after monocular occlusion during 0, 15, and 30 minutes, respectively.

Results: As the time of monocular occlusion increased, the lateral phoria of distance and near showed a change in the esophoria (p=0.017 for distance, p=0.007 for near). As a result of the analysis according to the type of lateral phoria, the esophoria direction changed only in the exophoria group for distance (p=0.002), while the orthophoria and exophoria groups showed a change in the esophoria direction for near (p=0.001 for orthophoria group, p=0.003 for exophoria group). The vertical phoria showed a change in the hyperphoria direction only at near (p=0.031), and only the hypophoria group showed a change in the hyperphoria direction at both distance and near (p=0.017 for distance, p=0.006 for near). In the lateral fusional vergence, the amplitude of base-out fusional vergence increased as the monocular occluded time increased in distance and near (p<0.001 for distance, p=0.005 for near). As for the vertical fusional vergence, only the amplitude of the base-up fusional vergence increased for near (p=0.036).

Conclusions: Monocular occlusion has been shown to affect the phoria and fusional vergence, and when prescribing a prism in clinical practice, monocular occlusion should be considered for at least 15 minutes for lateral phoria and 30 minutes for vertical phoria.
